I strongly recommend you to seek professional evaluation before making any determinations about treatment decisions. This brief survey is designed for adolescents (ages 12-18).

6-14: You don't appear to have the symptoms of Attention Deficit Disorder Inattentive type. Take Part II to see if you have the H (Hyperactive component) of ADHD.

14-20: You may have the Inattentive part of ADHD. It's in your best interest to get further testing/evaluating. Start nagging your parents.
